PHARMACY PROGRAM MANAGER

JOB SUMMARY

Under the direct supervision of the Sr Manager of Clinical Support Services, the Pharmacy Program Manager is responsible for managing and leading the pharmacy technician care team using our Core Values to foster strong positive relationships and drive performance. Promotes superior customer service and seamless program coordination

CORE FUNCTIONS

  • The Pharmacy Program Manager will provide direct operational supervision for pharmacy care team technicians to include the following:

    • Responsible for performance of the program

    • Active participant in the Care Integration Platform

    • Provide leadership, administrative and clinical oversight for program development, implementation and evaluation of the pharmacy care team technicians

  • Promote the mission, philosophy, goals, and policies of the organization through staff education

  • Collaborate with other Care Team staff to help plan and execute against network quality goals

  • Provides clinical oversight of the pharmacy care plan and medication alignment setup and maintenance provided by Pharmacy Technician

    • Identify opportunities for improved processes with continuous mindset toward efficiency and increased productivity

    • Accountable for operational productivity and quality outcomes for the pharmacy care team technicians through executing side by sides, 1:1’s and coaching individual team members to productivity goals

    • Responsible for planning staffing and succession plans to meet business needs for his/her assigned team

    • Accountable for working with managers and leaders to support continuous process improvement within assigned workflow areas

    • Serve as communication conduit for daily technician operations between the team, other departments and leaders

    • Understand management needs and applies consultative problem-solving skills for long-term and day-to-day issues

    • Maintain personal professional development

  • Clinical Responsibilities

    • Identify members appropriate for medication management services by use of targeted chronic conditions, level of care, and recognition of member’s disease specific and preventative measures, knowledge base or deficits in monitoring health, wellness and chronic conditions. Reviews and analyzes clinical indicators and whether there is any ‘gap’ in compliance that will result in member contact

    • Performs telephonic medication assessments utilizing prescribed workflows

      • Complete assigned appointments in accordance with workflow

      • Assess patient adherence behaviors

      • Eliminate barriers to adherence

      • Assess appropriateness of therapy provided by the pharmacy

      • Document appropriately in Care Management and Pharmacy programs

    • Develop and implement a pharmacy care plan, including medication alignment and dispensing

      • Follow all pharmacy procedures as part of medication alignment process which includes:

        • Accessing, inputting, and retrieving information through the Pharmacy’s computer network to maintain accurate records

          • Create accurate prescription labels

          • Obtain refill authorizations from prescribers

          • Process and prepare third-party insurance claims

          • Maintain accurate patient/ client profiles

          • Provide patient/ clients with counseling and education

          • Monitor storage, distribution, and use of pharmaceuticals

          • Document and report medication errors to the Pharmacist in Charge

          • Properly secure and maintain controlled substances

          • Operate the state prescription drug monitoring program

  • Perform other duties as assigned

QUALIFICATIONS AND EDUCATION REQUIREMENTS

  • Doctor of Pharmacy degree in pharmacy from an accredited institution

  • Licensed as a registered pharmacist with an unrestricted license in the state of Texas and be eligible for pharmacy licensure elsewhere

  • Knowledge of federal and state regulations regarding pharmacy

  • Possesses current knowledge of disease pathophysiology, psychosocial issues, and current pharmacotherapy for core chronic disease states

  • Immunization certified

  • Ability to handle multiple demands of diverse workload and prioritizes critical issues

  • Ability to build effective collegial relationships

  • Ability to influence and effect change

  • Ability to analyze and think critically

  • High level of integrity

  • Able to develop, implement, communicate and evaluate a plan of care for each call

  • Ability to maintain a HIPAA compliant professional work environment

  • Excellent oral and written communication skills

  • Excellent leadership and customer services skills

  • Strong organizational and time management skills

  • Computer skills as necessary to operate Pharmacy management software and other necessary programs

  • Must maintain valid driver’s license and vehicle

  • Ability to work a flexible schedule, including evenings and weekends

  • Ability to sit and stand intermittently for long periods of time

  • Ability to bend, reach, and type
